If your child requires an inhaler to be left in school, please can you complete Asthma Consent Form 1, 2 and 3.
If your child requires medicine to be administered during the school day, please complete Medicine Consent Form 1 and 2.
As per our Supporting Pupils at School with Medical Conditions Policy, medicines should only be administered at school where it would be detrimental to a child’s health if it were not administered during the day.
Medication provided needs to be clearly labelled with your child's name and forms will need to be signed off by the office staff before your child can be administered their medication.
